APPOINTMENT
- A credit card will be required to hold all appointments.
- When booking, the appointment program uses your credit card information to secure your appointment, and will only charge you for no-show, late cancellations or rescheduling (less than 24 hours’ notice). Zhiva Esthetics do not have access to your credit card information, and these charges are handled securely through Square Booking.
- Clients are subject to rescheduling if they are over 15 minutes late
- Unconfirmed appointments will be canceled 12 hours prior to scheduled appointments.
- We advise guests to arrive 5 minutes before their scheduled appointment time to allow for intake forms and to avoid scheduling issues.
- By scheduling your appointment, you agree to any charges that may occur based on a Cancelation/Late Cancelation/No Show Fee.
CANCELLATION/NO-SHOW
- Yor are always welcomed to cancel or reschedule your appointment 24 hours or more before is scheduled at No Fee.
- Less than 24 hours cancellations will be charged a $25 late cancellation fee.
- Same-day cancellations will be charged $50 for the services booked.
- No-shows will be charged the full amount for the service booked.
- Unconfirmed appointments will be canceled 12 hours prior to scheduled appointments.
- Clients are subject to rescheduling if they are over 15 minutes late.
AGE LIMIT
- All clients must be at least 14 years of age to receive treatment and clients under the age of 18 must be accompanied by an adult for the entire duration of their treatment.
CHILDREN
- Children over the age of 13 who can wait in our lobby during your treatment are welcomed.
- You cannot bring children under 13 with you to your appointment unless you have another adult to accompany you to watch your child in the waiting area.
SKIN, MEDICAL, ALLERGY CONDITIONS
- If you are pregnant or intending to be, please disclose this, so the facials can be modified to be pregnancy safe. It’s always advised to get clearance from your physician.
- If you have any allergies or any sensitivities to foods, medications or skin care, it is your responsibility to notify us of this before your treatment.
- If you have a health condition or are on medications or supplements that can impact healing, it’s strongly advised to have clearance from your treating physician and disclose this in your intake or to me in person.
- Certain issues can only be addressed by a dermatologist. Specifically, deep and painful acne lesions, cystic acne, acne that’s already started to scar, redness, or anything that’s scabbing and crusting. Additionally, anything that hasn’t already been diagnosed—rashes, dryness, sensitivity, and brown spots—should be seen by a dermatologist first.
PHONE USAGE
It’s recommended that you keep your phone on silent and out of reach during our appointment not only so you may relax, but so our neighbors getting massage in our space may relax as well.
PAYMENT
- Gift Cards/Certificates and Coupons are needed at the time of service to be redeemed. We cannot accept expired discounts or Gift Certificates.
- We accept major credit cards, cash and checks (with valid US Driver’s license). We can email a copy of your receipt if you prefer to have one for your files.
PRODUCT REFUND
Our policy is valid for a period of 30 calendar days from the date of the purchase.
The following criteria must be met to qualify for a refund:
- Product must be unopened
- Product must be in original packaging
- Product must be unused
- Product must not be damaged
If the product does not meet the listed criteria, we reserve the right not to issue a refund.
PROOF OF PURCHASE
To complete your refund, we require a receipt, purchase order or other proof of purchase. Please note that without the proof of purchase, we will not issue a refund.
SALE AND CLEARENCE TIMES
Only regular priced items may be returned, unfortunately sale or clearance items cannot be returned.
SHIPPING ITEMS
Returns can be mailed to: Zhiva Skin 36800 Sidewinder Rd., Unit B14, Carefree, AZ 85377. You will be responsible for paying for the shipping costs with regard to the items that you wish to return. We will not refund the shipping costs. You must take care to ensure that the goods are properly packaged so that they will not be damaged while in transit. If the product is found damaged or used beyond what it takes for us to reasonably inspect it, then we may reject a refund.
GIFT CARD TERMS AND CONDITIONS
VALIDITY PERIOD
- This Gift Card expires 24 months after the date of purchase.
REDEMPTION
- Redeemable only at Zhiva Esthetics for services currently offered, valued at the current selling price.
NON-REFUNDABLE
- This Gift Card is not refundable and cannot be returned or redeemed for cash or credit.
NON-TRANSFERABLE
- This certificate is non-transferable and may only be used by the original recipient.
LOST OR STOLEN CERTIFICATES
- Zhiva Esthetics is not responsible for lost or stolen Gift Cards. Replacement of lost or stolen cards is not possible.
REMAINING BALANCE
- This certificate does not allow for a cash refund on any unused portion of its value.
- In the event that the Gift Card’s value does not cover the full amount of the service or product, the bearer is responsible for paying the remaining balance.
EXCLUSIONS
- This Gift Card cannot be used in conjunction with any other promotional offers, annual sales or discounts.
CANCELLATION
- Zhiva Esthetics reserves the right to cancel this Gift Card if necessary due to circumstances beyond our control.
- Zhiva Esthetics reserves the right to cancel any Gift Card obtained fraudulently.
- Gift Cards may be voided by Zhiva Esthetics if any alterations or unauthorized endorsements are detected.
- In the event of a business closure or transition, outstanding certificates may be subject to cancellation.
- Zhiva Esthetics may cancel this Gift Card if it’s found to be part of an unauthorized resale transaction.
MODIFICATION OF TERMS
- Zhiva Esthetics can change the terms and conditions of the Gift Cards at any time without prior notice which will take immediate effect upon publishing online on our website.
JURISDICTION
- All disputes related to this Gift Certificate will be resolved under the laws of the state of Arizona.
